Applying for a CitizenCard

What you need to enable you to apply:

1. An Application Form

  • You can download an application form from the CitizenCard website (www.citizencard.com).
  • You can use an application form available from Swindon Borough Council Customer Service Desks, Swindon Libraries, secondary school offices and various retailers.
  • You can apply entirely on-line via the CitizenCard website on www.citizencard.com.

2. Consent

  • If you are under 16 you will need parental/guardian consent

3. A Referee

  • Every applicant’s details are verified with a referee before a card is issued.  You will need to provide the name, address and work phone number of a referee who can verify your details.
  • A referee must be one of the following: teacher, lecturer, doctor, dentist, bank manager, lawyer, magistrate, police officer, local authority, sheriff, minister of religion, civil servant, MP or any other professional person.
  • Retired persons, persons under 25 years old, persons working from home, a retailer of age-restricted products or a person related to you may not act as a referee.

4. Two passport-size photographs

  • The referee is required to sign the back of one of the photographs.

5. Payment

  • The CitizenCard is FREE of charge only through the Rolling School Programme. All other applications will cost £9.
  • If you apply entirely on-line using CitizenCard’s own website the cost is £9.
  • If you wish to fast-track your application for delivery in 5 working days or less, an urgent application surcharge of £24 can be paid. Please download a form from the CitizenCard Website (www.citizencard.com).

Sending Your Application

Application forms, photographs and any payments required must be returned to the CitizenCard Card Centre to be processed. The postal address is:
