Your application will need to be made on the Application for Subject Access form. If you have appointed an agent to act on your behalf, then you will also need to complete the agent authorisation form.

You can complete the application online form by following the link below. However, you will still need to print out the form, sign it and then send it to us (together with proof of ID and the fee) before any action can be taken. If you have appointed an agent, then he or she must also sign the form after it has been printed.
